If you've recently received a DUI or DWAI, you're probably asking the same questions everyone else asks:

Take a deep breath. The process can seem confusing, but it becomes much more manageable when you take it one step at a time.

 

The Three Most Common Requirements for License Reinstatement

Before the Colorado DMV will reinstate your driving privileges, most clients will need the following:

 

1. SR-22 Insurance: Your insurance carrier provides this. ​

2. Affidavit of Enrollment:  Integrity Health & Wellness provides this form.

The Affidavit of Enrollment verifies that you are actively enrolled in your required Level II DUI Education and/or Therapy program.

3. Ignition Interlock Installation This is provided by the interlock Company your choose  

If required by the Colorado DMV, your Ignition Interlock Device must be installed before your driver's license can be reinstated.

Do NOT drive your vehicle to the interlock installation appointment if your license is suspended. Ask a family member, friend, or another licensed driver to take your vehicle to the installation facility.

 

Additional Treatment Documentation

DRS Driving Record System (DRS) paperwork. After you have completed all of your required Level II Education and Therapy, Integrity Health & Wellness will provide this to you.  It is your responsibility to upload the DRS to the Colorado DMV. We do not submit DRS paperwork on behalf of clients. This ensures you have confirmation that your documentation has been received and allows you to track your reinstatement directly with the DMV.

 

Please note: Every case is different. Additional requirements may apply depending on your driving history and DMV determination. Only the Colorado DMV can determine your eligibility for license reinstatement.

 

⚠️ Important DMV Notice

The Colorado DMV may automatically send a form letter stating that you have not completed your required DUI Education and/or Therapy. This letter can be alarming, but it is often generated based on your estimated completion date in the DMV system.

 

Don't panic.

If you have completed your treatment with Integrity Health & Wellness, this notice usually means the DMV has not yet received your Driving Record System (DRS) documentation.

If you receive this letter:

• Verify that you have completed all of your treatment requirements.

• Upload your DRS to the Colorado DMV as soon as possible if you have not already done so.

• Keep a copy of your DRS and any confirmation that it was submitted.

• If you have questions about your treatment completion, contact Integrity Health & Wellness. If you have questions about your driver's license status, contact the Colorado DMV directly.
