About Integrity Health & Wellness

Integrity Health & Wellness, LLC provides behavioral health services, including DUI education and treatment services, intake services, and private therapy sessions.

 

Some clients participate in Integrity services to meet requirements associated with a court order, probation, driver's license matter, or another governmental or administrative process.

 

This website provides information about Integrity and allows visitors to schedule certain appointments. The website itself does not provide treatment or establish a therapeutic relationship.

Website Scheduling

The website may be used to schedule services including:

  • An initial informational call regarding DUI education and treatment services

  • An intake appointment

  • A private therapy session

 

Scheduling an appointment through the website does not guarantee acceptance into a particular program, establish eligibility for services, or guarantee that a service will satisfy a court, probation, DMV, or other governmental requirement.

 

No payments are processed through this website. Fees, payment requirements, and any applicable payment policies are communicated separately in connection with the services provided.

DUI Education & Treatment Services

Integrity provides DUI-related education and treatment services that may be undertaken in connection with court, probation, driver's license, or other requirements.

 

Requirements can vary according to an individual's circumstances. Integrity will provide information and documentation concerning a client's participation and completion of services when appropriate.

 

However, each client remains responsible for understanding and satisfying the requirements applicable to their individual case.

 

Integrity cannot guarantee that participation in or completion of a particular service or program will satisfy the requirements of a court, probation department, the Colorado Division of Motor Vehicles, or any other governmental, regulatory, or administrative authority.

 

Clients should communicate directly with the appropriate authority when clarification of their individual requirements is needed.

Attendance & Missed Classes

Regular attendance is an important part of DUI education and treatment programs and may be necessary to complete program requirements.

Clients who miss a required class may be required to complete a makeup class. Integrity currently provides designated makeup-class opportunities twice each year.

Availability, scheduling, and eligibility for makeup classes may depend upon the program and the client's individual circumstances. Clients are responsible for communicating with Integrity regarding missed classes and any requirements necessary to complete their program.

Private Therapy Services

Integrity also offers private therapy sessions with qualified providers.

Scheduling a therapy appointment through this website is an administrative action only. A therapeutic relationship is established through the appropriate intake, consent, disclosure, and professional-service process—not simply by visiting this website, submitting information, or scheduling an appointment.

Therapy services are provided according to applicable professional standards, informed-consent requirements, confidentiality requirements, and Colorado law.

Service For Minors

Integrity may provide services to individuals under 18 years of age.

Consent, authorization, parental or guardian involvement, confidentiality, and disclosure requirements for minors will be handled according to the type of service being provided, the circumstances of the individual client, applicable professional requirements, and Colorado law.

Nothing in these Terms is intended to expand or restrict the rights of a minor, parent, guardian, or behavioral health professional provided by applicable law.

Emergency & Crisis Disclaimer

This website and its scheduling system are not intended for emergencies or crisis intervention.

Do not use the website, email, contact forms, or appointment-scheduling system to seek emergency assistance.

If you or another person is experiencing an immediate medical or mental-health emergency, is in immediate danger, or requires urgent assistance, call 911 for immediate danger or medical emergency, or call/text 988 for a mental-health or suicide crisis.

Messages and appointment requests submitted through the website may not be reviewed immediately.
